Once you have read through the Guidance notes and the Grant application requirements, please follow the instructions below to access and submit the application form:

1. Access the grant application form from this link. 
2. 'ENABLE' the form before downloading it to your device - otherwise you will not be able to fill in the tabs.
3. Determine your eligibility by answering all the questions. If you have answered 'Yes’ to all the questions, you may continue completing the form.
4. Save the completed form on your device and email it to grants@stolavesfoundation.co.uk together with the requested supporting documents.

The information provided in this application form will be stored and used by the Charity in accordance with the Data Protection Act 1998.
